Camp Application

Although supervision and instruction will be provided to mitigate against injury, I recognize and am aware that participation in a technology camp and in laboratory settings could involve risk of injury. I have voluntarily enrolled my child for this activity. I understand and agree that West-MEC and their employees will not be held liable for any losses, injury or damages occurred as a result of my child's participation. My child does not have any physical conditions which will prevent him or her from participating in the camp. I consent to the use and release to West-MEC, the use of my student's name or likeness in still photographs or videos for future marketing materials or other uses as they see fit. Furthermore, I am aware that there are no refunds, or credits of fees after fees have been paid.

Transportation provided by parent - please drop off/pick up your child within 15 minutes of camp start/end time.
